It’s no longer officially scientific to recommend a PSA test “just to see”. A U.S. Preventive Services Task Force is preparing to recommend that the PSA test get a “D” rating, meaning the results are unreliable and likely to lead to unnecessary biopsies, in which the risks outweigh any measurable benefit.
The PSA test results in “small or no reduction” in prostate cancer deaths. So what’s the point?
Of course those who have a personal point of view are not much interested in the scientific reality. A spokesman for the Prostate Cancer Foundation, Dan Zenka, was reported by CNN as describing the proposed recommendation as “a tremendous mistake. You’re talking to someone whose life was saved by [the PSA test].”
But Dr. Kenneth Lin, senior author of the new paper, said he believes PSA testing does more harm than good. “Maybe you should get tested if you have this horrible family history where everyone gets prostate cancer before the age of 50. But for most men, testing is harmful,” he said.
Prostate cancer is the most common type of cancer found in American men, other than skin cancer. It’s the second leading cause of cancer death in men, behind only lung cancer.

What gets lost in all this noise, which has reached a crescendo, are the hard facts about cancer screening. Let’s look at the latest scientific research on the pros and cons of one of the most controversial tests: mammography for breast cancer.
Breast Cancer Is Overdiagnosed and Overtreated
Approximately 70 percent of women over age 40 report having had a recent mammogram. They accept the pain of the procedure, which I’ve heard jokingly referred to as like lying down on the driveway and letting a car run over your breast. They overlook the radiation exposure, which adds up with annual screenings and itself increases risk of cancer. Why? Because they’ve been convinced it’s their best defense against death from breast cancer. But is it?
Recent studies completed by the Cochrane Collaboration, a well-respected international organization that reviews the science behind various medical interventions, looked at all the randomized clinical trials that compared routine mammography with no screening. The data revealed that screening mammography conferred a 15 percent reduction in risk of death. But it also led to 30 percent overdiagnosis and overtreatment, and the absolute risk reduction was just 0.5 percent. The researchers concluded, “It is thus not clear whether screening does more good than harm.”
The Dark Side of Screening
Furthermore, for every 100 women screened, more than 10 of them will have a false positive, which usually leads to significant psychological distress. Emotional trauma aside, false positives open the door for biopsies and other tests, as well as treatment of tumors that would never amount to anything. The Cochrane group reported that significantly more of the women who had been screened had lumpectomies and mastectomies.
The knee-jerk reaction is to assume that these procedures saved lives. Ask any woman who has undergone such a procedure, and she’ll likely agree. But she may well have had an “indolent” cancer, which studies suggest frequently regress and disappear on their own. These are the tumors that are far more likely to get picked up on yearly screening, not the aggressive, rapidly spreading cancers that are responsible for most breast cancer deaths.
Don’t Be Scared Into Screening
As you can see, the scientific data on screening mammography is quite murky. Routine screening for women who are in their 40s or older than 75 has not been demonstrated to provide significant benefits. For asymptomatic women between the ages of 50 and 74, benefits are scant, and in my opinion, are outweighed by the risks of overdiagnosis and overtreatment.
Otis Brawley, Chief Medical Officer of the American Cancer Society, was quoted in the New York Times as saying, “We don’t want people to panic, but I’m admitting that American medicine has overpromised when it comes to screening. The advantages to screening have been exaggerated.”
Am I telling you not to be screened? No. But I am telling you that the evidence of benefit is slight to nonexistent and in no way warrants the scare tactics that are widely used. Do your research and make your own decision based on facts, not fear.

A strain of harmless bacteria that live in soil could soon be helping to kill cancer tumors, thanks to researchers from the University of Nottingham in the UK and the University of Maastricht in the Netherlands who are presenting their work at a conference in York, England, this week. They said they expect to test the strain in cancer patients in 2013, and if successful, hope the method can be combined with additional approaches to win the battle against cancer.
The bacterium is Clostridium sporogenes, which is widespread in soil. Injected into cancer patients, the bacterium grows in solid tumors and releases an enzyme which triggers a separately injected “pro-drug” to kill cancer cells.
The anti-cancer drug only becomes active when it meets the bacterial enzyme in the tumor and therfore only targets cells nearby.
To take the experimental therapy from the lab stage to be ready for clinical trials, the researchers had to carry out a number of improvements.
One improvement was to insert a gene into the bacterium’s DNA that made it produce greater quantities of the trigger enzyme in the tumor. This also improved the enzyme’s ability to convert the “pro-drug” into its active form.

According to a 2000 survey, Americans’ worst fear is a cancer diagnosis. One out of every three women and one out of every two men will be diagnosed with cancer in her or his lifetime.
It’s against this backdrop that the American Association for Cancer Research, which bills itself as the world’s largest cancer research organization, posed the question: Where do we stand in the war against cancer?
The results, published in a new progress report urging Congress to increase funding for cancer research, are complex.
According to the report’s authors, in 2010, more than 570,000 people died of cancer in the United States. There are also currently more than 12 million cancer survivors, and mortality rates have declined steadily since 1990.
“Personally, I think it’s a remarkably hopeful time,” said Dr. Judy Garber, director of the Center for Cancer Genetics and Prevention at Dana-Farber and president of the AACR.
“The vast majority of things I do for patients now are based on research done in labs very recently,” she continued. “The pace of discovery and translation have both accelerated.”
One key, according to the 84-page report, has been researchers learning that cancer is not a single disease, but more than 200 different ones. They also now better understand the basic biology of cancer, including the crucial role gene mutations play.
The AACR’s new progress report also highlights the great strides that have been made in drug research, particularly in the development of drugs that target tumor cells, with fewer side effects, that the report states are “much less toxic.”
Prevention and public health efforts have also been important, the authors write.
Reduced smoking has saved lives that would have been lost to cancer — not only lung, but also 17 other types directly related to tobacco use. Yet the report also finds that at least half of all cancers and cancer deaths could be prevented by changing personal behavior, including things like improving diet and avoiding a sedentary lifestyle.
Indeed, in an interview with the HuffPost, Garber called for continued public health efforts to help highlight the role that prevention can play in combatting cancer, as well those aimed at promoting and improving early detection.
“This is a solvable set of diseases,” she said, saying that increased funding for cancer and biomedical research is essential. The report recommended Congress increase the National Institute of Health and National Cancer Institute’s annual budgets by at least 5 percent beyond the biomedical industry’s inflation rate. “We are poised on a threshold.”
